Please fill out the QPHA Annual High Point Award Nomination Form as well when sending in membership form.

If cards are full please print extra High Point Sheets off from here and attach to inside of your High Point Card.
You must attend a minimum 5 shows/events to be eligible for High Point Awards.
You can pay your fees by direct deposit.
